1. Official transcript of First semester marks, grade 12.
2. A letter indicating your financial need and future plans.
3. List your participation (IN SEPARATE COLUMNS) in your school, community, church and
sports.
4. TWO letters of reference, one required from a teacher or school principal. One required
from a respected person.
